As an LPN in Long Term Care, you will be a significant customer service asset when catering to the needs of residents. Your nursing skills apply in conducting patient assessments, planning individualized care, delivering treatments, and evaluating outcomes. Collaborating with interdisciplinary teams, family members, and social supports, you will advocate the welfare of the residents.

Job Description
Essential Functions:
• Appropriately delivers medications and vaccinations based on policy and their authorized domain of practice.
• Adequate resources will be provided in patient care areas as needed, and the assistant will ensure that items are restocked promptly.
• Provide support to the Medical Assistant or Clinical Assistant in communicating effectively with patients and their families regarding treatment options.
• The assistant collaborates with the RN to gather necessary data during the admission assessment process by auditing, observing, and reviewing charts while adhering to age-appropriate criteria.
• The assistant is responsible for assisting the registered nurse in every stage of the nursing process, from initial assessment to ongoing evaluation, ensuring that patients receive high-quality, comprehensive care.
• Providing timely and accurate updates to the Interdisciplinary Plan of Care supports communication, collaboration, and coordination among healthcare professionals
• Keeps patient throughput seamless and orderly by prioritizing care based on individual needs and making swift, confident decisions.
• Shows expertise and acumen necessary for implementing physician orders, medical practices, and nursing interventions.
• Multidisciplinary relationships can lead to more creative solutions by allowing individuals to leverage their unique skills and backgrounds.
• Instructs the individual, their loved ones, and the healthcare staff in the treatment regimen and objectives.
• Contributes to the appraisal, planning, execution and review of healthcare in various intricate situations with reliance on evidence-based care.
• Prompt, correct and suitable documentation of clinical care offered to patients is generated.
• Any other duties as instructed.
Additional Requirements
Required:
Education – Completion of a nursing education program that is approved by a board of nursing
Certifications – Basic Life Support (BLS) Certification – American Heart Association (AHA); Licensed Practical Nurse (LPN) – South Dakota Board of Nursing
Preferred:
Experience – 1+ years of Healthcare Experience; 1+ years of Licensed Practical Nurse Experience
Physical Requirements:
Very Heavy work – exerting over 25 pounds of force constantly (67-100% of the time), and/or more than 50 pounds frequently (34-66% of the time), and/or more than 100 pounds of force occassionally (up to 33% of the time), and/or more than 100 pounds of force seldomly to move objects. Possible exposure to radiation and frequent exposure to contaminated needles and infectious body substances. Requires the ability to work designated shift lengths (including 8 and 12 hours), which may include night shifts or occasional irregular hours.
