The Opportunity
• Provides case management to a group of patients providing specific assessments according to protocols and procedures, clinical competence in procedural skills, treatments, and patient/caregiver education related to wound care management.
• Organizes and prioritizes clinical responsibilities of clinical staff including medical assistants and wound care technicians
• Collaborates with other health care providers, key hospital personnel, Wound Care Center Providers, and Medical Director regarding clinical and patient needs.
• Consistently looks at current processes and procedures, and identifies and implements areas of improvement, while complying with Health Insurance Portability and Accountability Act (HIPAA) and the Department of Health (DOH).
• Calls patients to communicate lab and test results.
• Develops and maintains a system of “care consideration” of our primary care patients to be sure the front office schedules the patient to be seen for appropriate tests/therapies.
• Engages in staff development, providing general in-services as well as individual instruction and orientation.
• Provides staff recognition through positive reinforcement and constructive feedback.
• Ensures that the clinic adheres to all relevant healthcare regulations, standards, and guidelines.
• Implements quality assurance measures to monitor and improve the quality of patient care.
• Conducts regular audits and assessments to identify areas for improvement and address any compliance issues.
• Collaborates with Medical Director to achieve Wound Care Center quality indicators.
• Performs patient care based on Center staffing needs
• Collaborates with Medical Office Coordinator and Medical Director with all-staff meeting and actively participates in ongoing provider meetings.
• Ensures daily operations and steady progress towards established goals.
• Establishes and maintains open and effective communication with all members of the staff.
• Identifies problems and communicates solutions to appropriate personnel and departments with ongoing communications in conjunction with the Clinic Manager and Medical Director.
• Identifies opportunities for process improvement and efficiency gains within the clinic; partners with manager and MD to implement as appropriate.
• Implements new technologies or workflows to enhance patient care delivery and operational effectiveness.
• Helps ensure a safe patient and work environment by maintaining incident reports, monitoring room hygiene logs, inventory/medication expiration date logs, skin substitute logs, etc.
• Monitors Wound Care Tech/Medical Assistant tasks related to patient inquiries, complaints, and feedback.
• Responds to patient complaints and questions.
• Maintains employee counseling records for clinical staff and liaises with supervisor regarding clinic issues.
• Monitors clinical skills of clinical staff.
• Delivers clinical education training programs, as needed.
• Covers the front office as needed based on staffing levels. Responds to and/or replaces back office call outs.
• Monitors patient flow and assists clinically as needed based upon wait times and needs of the clinic.
• Assists the management team with maintaining equipment and calibration records.
• Monitors inventory levels and orders supplies as needed to ensure adequate stock levels.
• Participates in Outpatient Medicine and Vituity projects to accomplish the strategic plan of Vituity.
• Orients and educates staff as needed on expectations, clinic policies, etc.
Required Experience and Competencies
• Current active Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) in the state of practice is required.
• Two (2) years of direct patient care experience required.
• Current Basic Life Support (BLS) certification is required.
• Clinical nursing experience with a focus on wound care preferred.
• Experience working in an acute care or rehabilitation setting preferred.
• Proficient in all responsibilities related to Case Management.
• Strong clinical assessment and critical thinking skills.
• Effective communication and interpersonal skills. Ability to develop and maintain effective relationships with patients, families, staff, providers, and all other members of the health team.
• Ability to work collaboratively with our team.
• Proficient with advanced wound care technologies and techniques.
• Solid understanding of medical insurance plans and office procedures.
• In-depth knowledge of medical documentation
• Strong clinical assessment and critical thinking skills.
• Ability to demonstrate leadership skills to delegate and provide direction/guidance to others on team.
• Able to stay organized and flexible in a work environment with multiple assignments and changing priorities.
• Ability to uphold Health Insurance Portability and Accountability Act (HIPAA) requirements.
• Highly organized and able to manage multiple priorities.
• Ability to identify strategies to improve patient satisfaction and outcomes.
