• Provide and coordinate social services for patients of all ages in their homes
• Advocate for patients encountering psychological, health, family, and financial struggles
• Assess mental and emotional stability by understanding the social and financial conditions of patients and their families
• Collaborate with healthcare workers to deliver optimal healthcare services
• Ensure thorough and timely electronic documentation and interdisciplinary communication, adhering to agency policies and standards of practice
