OverviewThe Heart Failure Nurse Navigator position serves adult patient populations that will be treated within Cone Health, on the Moses H. Cone campus. The position acts as the primary resource for staff, patients and families for all information and education. The HF Navigator works collaboratively with the HF multi-disciplinary team to ensure that patient care meets established quality standards.
