OverviewThe Heart Failure Nurse Navigator position serves adult patient populations that will be treated within Cone Health, on the Moses H. Cone campus. The position acts as the primary resource for staff, patients and families for all information and education. The HF Navigator works collaboratively with the HF multi-disciplinary team to ensure that patient care meets established quality standards. The navigator uses recognized heart failure metrics to continually improve quality patient care and to understand readmission trends.Talent Pool: NursingResponsibilitiesContributes to the strategic objectives of the HF Program, to include the following: LVAD Program, Paramedicine Program, Transitions of Care, Inreach and Outreach————————————————–Coordinates care among multiple specialties————————————————–Ensures continuity of care throughout the patient stay————————————————–Facilitates post-discharge appointments, and performs post-discharge phone calls, when appropriate————————————————–Identify patients admitted with a diagnosis of HF————————————————–Liaison to the AHF Clinic/HF Team————————————————–Participates in weekly multi-disciplinary HF team meetings————————————————–Partners with THN counterparts to achieve HF goals/objectives————————————————–Performs daily rounds, and provides patient and staff education; services as patient and staff resource————————————————–Serves as patient and physician point of contactQualificationsEDUCATION:Bachelors, Nursing, PreferredEXPERIENCE:Required:Minimum of 2 years’ of heart failure care experiencePreferred:Prior experience working with heart failure patientsLICENSURE/CERTIFICATION/REGISTRY/LISTING:REQUIREDRN | Registered Nurse licensed in North Carolina or a Compact state.Drivers License | Valid Drivers LicenseBLS (CPR)-American Red Cross or AHA Healthcare Provider
