Patient Care Coordinator
LHH Recruitment Solutions is currently seeking a full-time patient care coordinator for a client in the Sorrento Valley, CA area on a contract to hire basis. The client requires the representative to ensure that patients receive the necessary support, resources, and services to facilitate their healthcare journey.
This position is in Sorrento Valley, CA and requires the candidate to live in or near Sorrento Valley.
Compensation: $18/hr
Job Type: Contract to Hire
Schedule:Monday – Friday, 8a – 5p, varies by location
Responsibilities:
• Serve as a patient advocate by ensuring that patients’ needs, and preferences are communicated and addressed effectively throughout their care.
• Coordinate and facilitate the continuity of care by collaborating with healthcare providers, specialists, and other members of the healthcare team to ensure appropriate and timely delivery of services.
• Manage patient appointments, including scheduling and rescheduling as necessary, and coordinate with healthcare providers and patients to minimize scheduling conflicts.
• Ensure accurate and up-to-date maintenance of patient records, including medical histories, test results, treatment plans, and other relevant documentation.
• Coordinate referrals to specialists, diagnostic tests, and procedures, obtain necessary authorizations from insurance providers, and ensure seamless transitions between different healthcare settings.
• Provide patients and their families with information regarding their medical conditions, treatment options, and resources available to support their healthcare needs.
• Assist patients in understanding their insurance coverage, and eligibility for financial assistance programs, and facilitate the completion of required documentation.
• Act as a point of contact for patients, addressing their questions, and concerns, and providing emotional support during their healthcare journey.
• Collaborate with healthcare professionals to identify areas for improvement in patient care processes and participate in quality improvement initiatives.
• Ensure compliance with relevant healthcare regulations, privacy laws (such as HIPAA), and organizational policies and procedures.
Qualifications:
• High school diploma/GED is required.
• 1+ year of experience maintaining metrics, required.
• Experience working in a high-volume environment, required.
• Experience managing others, required.
