Serves as the primary contact supporting individuals in the coaching program with Developmental Disabilities and Mental Health challenges for the START Program (Systematic, Therapeutic, Assessment, Resources, and Treatment) to provide community based intervention and prevention services to active participants within START. The program relies on integrated care and treatment with collaboration among all team members. The services in the START Program include collaborative contacts, after hour contacts, emergency team meetings, therapeutic coaching and therapeutic respite.

RHA is seeking an organized and positive for Therapeutic In-Home Coach our START program. Serves as the primary contact supporting individuals with Developmental Disabilities, Mental Health or Substance Abuse Issues for the START Program (Systematic, Therapeutic, Assessment, Resource, and Treatment) to provide community based intervention and prevention services to individuals with developmental disabilities and behavioral health care needs.
The program relies on integrated care and treatment with collaboration among all team members. The services in the START Program include collaborative contacts, after hour contacts, emergency team meetings, planned respite and emergency respite.
Our ideal START Therapeutic In-Home Coach is an excellent communicator and cooperative team player. You must also be detail-oriented and have excellent written communication skills to develop and disseminate person-centered plans.

Education:
• Bachelor Degree in Psychology, Social Work or other Human Service related field required with 2 years experience with population served preferred
Populations Served Include: Intellectual Developmental Disability, Mental Health (Child and Adult)
Essential Job Related Responsibilities
• Coaches work within the home or other community settings to create and implement person specific and crisis therapeutic interventions and communicate these interventions with primary caregivers.
• Coaches contribute to the overall crisis stabilization and prevention of the individual and family by providing hands-on assistance and education to families.
• Coach participates as part of comprehensive START services that collaborate with individual’s primary provider and clinical home, community crisis services, family and others.
• Offer consultation and hands-on education with entire family support system to create a safe, nurturing, and meaningful environment for families and individuals with developmental disabilities
• Assists individuals with implementation of daily activities, behavioral health needs, and health, wellness, and leisure activities.
• Monitors individuals for any mental health or behavioral symptoms and provides crisis prevention, intervention and de-escalation supports in accordance with crisis guidelines and person’s plan
• Maintains timely documentation of session and visits notes and other data
