Job Summary

As an LPN within Long Term Care, you will be an active part of a relationship-centered care team delivering safe, efficient, and effective care to the residents. You will utilize your nursing skills in the assessment, planning, implementation, and evaluation to provide individualized patient care. You will be an advocate in the care of individuals in conjunction with interdisciplinary teams of healthcare providers, family members and support systems.

Job Description
Essential Functions:
• Dispenses and administers drugs and vaccines with accuracy, adhering to policies and their professional boundaries.
• Adequate resources will be provided in patient care areas as needed, and the assistant will ensure that items are restocked promptly.
• Work alongside the Medical Assistant or Clinical Assistant to identify potential complications and take appropriate preventive measures.
• Assisting the RN with the admission assessment process, the assistant collects significant data by reviewing charts, observing patients, and conducting audits while following age-appropriate standards.
• Through their contributions to the nursing process, the assistant helps to ensure that care plans are tailored to meet individual patient needs, resulting in improved health outcomes.
• Providing timely and accurate updates to the Interdisciplinary Plan of Care supports communication, collaboration, and coordination among healthcare professionals
• Adroitly maintains the pace of care by accurately assessing the urgency of patient needs, managing schedules, and making informed decisions based on case priorities.
• Exhibits skill and caution essential in implementing physician directions, medical interventions, and nursing plans.
• Multidisciplinary influences can promote a more holistic approach to problem-solving and lead to better outcomes.
• Provides proper instruction to the individual, caregiver, and healthcare unit regarding the treatment plan and its objectives.
• Engages in assessment, planning, implementation, and evaluation of healthcare in multiple challenging scenarios while utilizing evidence-based best practices.
• Accurate and relevant clinical documentation pertaining to patient care is generated without undue delay.
• Any additional tasks as required.
Additional Requirements
Required:
Education – Completion of a nursing education program that is approved by a board of nursing
Certification – Licensed Practical Nurse (LPN) – South Dakota Board of Nursing
Preferred:
Experience – 1+ years of Healthcare Experience
Physical Requirements:
The position demands a high level of physical capabilities, with responsibilities calling for constant heavy physical exertion, exceeding 25 pounds 67-100% of the time, occasionally ensuring exerting a force of more than 50 pounds, up to 34-66% of the time, rarely necessitating the application of force above 100 pounds, approximately 33% of the time, or significant amounts of force to move objects. Additionally, expectation of exposure to radiation and frequent exposure to infected needles and contagious body contents exists with this job. The role accommodates shift designs of 8 and 12-hour shift hours, which may involve night shifts or occasional unusual working hours.
