Medstar Health is looking for a Community Health Outreach Manager to join our team at MedStar Southern Maryland Hospital Center!
As a Community Health Outreach Manager, you will oversee community benefit/community health activities and identifies, develops, and/or sustains working relationships with public, private, and faith-based partners. The outreach manager will identify, test, and implement recent programs and services that strengthen the hospital and community relationships. Special emphasis will be placed on cultivating community-based interventions that optimize the community health needs assessment results and the health and well-being of discharged patients. The manager also establishes and manages institutional procedures and practices to capture, document, validate, and report community benefit activities per IRS guidelines.

Primary Duties:
• Oversees population and community health activities, ensuring that services and contributions support the hospital’s Community Health Assessment Implementation Strategy. Interacts with various internal and external stakeholders to assimilate activities and resources identified by these groups.
• Implements evidence-based community health programming; tests and evaluates new concepts, as well as promising practices. Collaborates with various hospital departments to capture and validate community benefit qualifying activities; establishes completion deadlines and documentation standards. Manages input of all community benefit activities in the Community Benefit Inventory for Social Accountability (CBISA) software.
• Coordinates Community Health Advisory Task Force meetings and maintains routine communications with members; identifies new community partners to strengthen the effectiveness of the department. Writes annual community benefit IRS narratives and completes routine or ad hoc reports on the performance of the hospital’s community health activities.
• Collaborates with the marketing department to publicize initiatives sponsored by the department. 9. Oversees the Community Health Advocate Program. Coordinates programs, orders, and maintains an inventory of community health educational materials.
• Oversees and manages departmental initiatives and programs. Partners with philanthropy to secure funding that will advance the outcomes of hospital priorities and the Community Health Assessment Implementation Strategy.
Qualifications:
• Bachelor’s degree in public health, nursing, or related field.
• 1-2 years of progressively more responsible job-related experience.
• Certified Health Education Specialist preferred.
• Master’s degree public health, nursing, or related field preferred.
• Grant writing experience preferred.
