Job Summary:
In your pivotal role as Community Health Worker, you will provide peer-to-peer information, support, and education to patients working to manage health conditions and challenges. Additionally, through individual meetings, group events, and user-friendly materials, you will help patients maximize their self-care and make appropriate use of medical services and community resources; you will also model & exemplify good health habits. This role will be in collaboration with emergency medicine providers and social workers.
REQUIREMENTS
High School Diploma or GED
Community Health Worker Certification preferred; if no certification, you will need to be open to completing the CHW Certification program thru VU school of nursing at no cost to you. Must have reliable transportation to support off-site patient visits; mileage reimbursement will be provided.
