We are seeking applicants in the Human Services field, living in the areas of Worcester and its surrounding towns, who are interested in, and have experience in Case Management-related work, in support a youth and adolescent population. Case Managers should expect to work in a local afterschool youth program setting, and would be focused on the administrative duties of case management for the youth, in addition to handling any outreach, on-boarding, mentorship, and overall community engagement. In this role you will be responsible for providing support, guidance, and advocacy to young individuals who may be facing various challenges, such as homelessness, family conflict, substance abuse, or mental health issues. You will work closely with the population to assess their needs, develop individualized plans, and connect them with resources and services to promote their well-being and independence. Additionally, you will collaborate with other professionals and community organizations to ensure comprehensive support for youth and their families.

Responsibilities – Case Management
• Assist youth and adolescent populations with administrative duties related to their cases
• Coordinate resources for individuals in the program
• Connect clients with relevant resources and services, such as housing assistance, educational programs, mental health counseling
• Advocate on behalf of clients to ensure their rights and needs are addressed within various systems, including legal, educational, and healthcare settings
• Provide crisis intervention and emotional support to youth experiencing acute distress or instability.
• Offer ongoing support, mentorship, and encouragement to help clients navigate challenges and build resilience.
• Build and maintain collaborative relationships with community partners, including social service agencies, schools, healthcare providers, law enforcement, and youth-serving organizations.
• Maintain accurate and up-to-date case records, documenting client assessments, progress notes, service plans, and outcomes.
• Prepare reports and documentation as required by funding sources, regulatory agencies, and organizational policies.
