Lake Granbury Medical Center’s care manager coordinates the overall interdisciplinary plan of care for a patient, from admission to discharge. The care manager is expected to act as a liaison between patient/family and healthcare personnel to ensure necessary care is provided promptly and effectively. Care managers rely on experience and judgment to plan and accomplish goals and perform a variety of tasks. A certain degree of creativity and latitude is required. The care manager typically reports to a head of a department/unit.
Education
Associates degree required, bachelors degree preferred.
Experience
Must be a registered nurse with at least 2 years of clinical experience. Familiar with standard concepts, practices, and procedures within a particular field.
Licenses And Certifications
License to practice nursing in Texas
American Heart Association (AHA) or American Red Cross (ARC) BLS
Certification as a Case Manager within 3 years of hire
