Care Integration Coordinator for the ED at Providence Portland Medical Center, in Portland OR.
Part-Time/Day Shift
The Crisis Intervention Coordinator collaborates with the patient/family, the acute multidisciplinary team, and post-acute providers to implement the patients discharge plan. The CIC also proactively escalates risks and barriers to discharge to the care team; documents all discharge planning efforts in the patient’s medical record in a timely manner; maintains ongoing communication with the multidisciplinary team and follows all regulatory requirements and ministry polices regarding care coordination/discharge planning.

Required Qualifications:
• Coursework/Training: Psychology, social work, social services or Bachelor’s Degree Completed Bachelors degree
• 2 years of experience in health care setting (acute or post-acute)
• 1 year of experience working directly with psychiatric and/or substance use disorders
