WellStone is North Alabama’s most comprehensive behavioral healthcare provider serving over 13,000 persons a year in Huntsville and Cullman. We are a non-profit community organization that offers a wide range of services for every phase of life, from children to older adults.
We are hiring for a Care Coordinator in Huntsville, AL.
The shift is 8am to 5pm, FT, Mon – Fri
What We’re Looking For
• HS/GED required for the position with at least 2 years experience. All experience must include working with individuals with psychiatric, substance use, and/or developmental disabilities
• Associate’s/Bachelor’s/Master’s degree in a human service-related field preferred
• Excellent oral and written communication skills in the English language; Spanish helpful
• Computer literacy including use of Microsoft Office, internet applications, and use of Electronic Health Records (EHR)
• Valid AL driver’s license, good 5-year automotive record, reliable transportation
What You’ll Be Doing
• Follow up with clients regarding appointments, completion of forms, returning forms or receipts and other similar activities.
• Contact consumers who have missed appointments in order to re-engage them and promote recovery efforts.
• Working collaboratively with client/family, focused on supporting client’s movement toward recovery/resiliency goals.
• Aid in accessing public services, including financial, housing, emergency assistance, food, etc.
• Maintain contact with clients who are hospitalized, participating in discharge planning and communicating with all involved providers in developing aftercare plans.
• Other duties as assigned.
