Job Summary
Provides quality education, counsel, and support to clients and their significant others regarding diabetes self-care. Documents the education process, patient information and clinical recommendations according to department/program/regulatory guidelines, and measures outcomes to maximize continuity of care between settings. Participates in projects related to area of specialty and the development/revision of procedures/policies, educational materials/guidelines, educational programs. Collaborates with and serves as a resource for other health professionals to promote delivery of quality services and patient education within the institution as well as the community.
Essential Functions
• Assesses the educational needs of patients based on current disease status, other comorbidities, self-care behaviors, medications, laboratory data, lifestyle, factors that affect learning, and available resources.
• Develops and facilitates the implementation of individualized education plans developed mutually with the patient, integrating assessment data and determining the patient’s self-management goals.
• Provides education on essential content and skills and empowers patients/significant others for effective diabetes self-management, adapting materials and teaching approaches appropriate to the learners’ age, culture, learning style, and abilities. Evaluates learning and adapts teaching methods.
• Provides behavior intervention, helping patients identify barriers and facilitates patients’ problem-solving and coping skills for effective self-care behavior and behavior change.
• Communicates and collaborates with other members of the health care team and patients, in order to ensure ongoing care and coordination of chronic disease management.
• Makes medication recommendations to patient and physician for clinical diabetes management.
• Ensures that services are provided in accordance with State and Federal regulations, organizational policy, and accreditation/compliance requirements. Documents education process, patient information, and clinical recommendations according to department/program/regulatory guidelines.
• Engages in continuous study of the entire professional field, including best practices, to maintain the professional competence, knowledge, and skills necessary for the satisfactory performance of all assigned responsibilities. Completes required continuous training and education, including department specific requirements.
• Functions within the ambulatory setting.
Qualifications
• Bachelor’s Degree in Nursing or related healthcare field
• 3 years of recent relevant clinical experience including prior teaching experience with adult clientele required
• Registered Nurse (RN) – State of Michigan , or Registered Dietitian (RD) – Commission on Dietetic Registration – CDR required
• CPR – AHA American Heart Association required within first 90 days
• Certified Diabetes Educator (CDE) – National Certification Board for Diabetes Educators (NCBDE) required within first 3 years
Preferred Qualifications
• Masters Degree in related field
• Certified Diabetes Educator – National Certification Board for Diabetes Educators (NCBDE) upon hire
• 5 years of experience as a Diabetes Educator
