Job Summary
Provides outpatient HIV medical case management services and patient advocacy through education, counseling, advocacy, benefits assessment and enrollment, and coordination of services. Ensures patient access to essential medications, medical care, and support services. Provides services to people living with HIV/AIDS who are medically managed through the grant-funded Ryan White Program – Division of Infectious Diseases. All team members are expected to be knowledgeable and compliant with Palmetto Health-USC Medical Group’s values of compassion, dignity, excellence, integrity, and teamwork.
Accountabilities
• Performs medical case management functions for patients/clients of the Ryan White Program. Provides treatment adherence counseling to ensure readiness for, and adherence to, complex HIV/AIDS treatments.
• Responsible for intake and assessment of service needs.
• Develops a comprehensive, individualized service/action plan.
• Coordinates services required to implement the plan.
• Responsible for client monitoring to assess the efficacy of the plan.
• Responsible for benefit assessments and enrollment, e.g. Medicaid, Medicare, ACA, and ADAP.
• Provides coverage in the duty room to assist clients with Patient Assistance program applications.
• Serves as a member of the interdisciplinary health team.
Supervisory/Management Responsibilities
This is a non-management job that will report to a supervisor, manager, director or executive.
Minimum Education
Bachelor’s degree from an accredited School of Social Work or Bachelor’s degree in a related field.
Minimum Experience
One year experience in health or social services related area.
Other Required Experience
Psychosocial intervention skills. Analytical and problem solving skills. Strong written, verbal and telephone communication skills. Ability to work within interdisciplinary setting.
Licensed Bachelor of Social Work (LBSW) preferred.
