Schedule: Full Time
Shift : Days, Mon-Fri, approximately 8am-5pm. Weekends as needed.

The role you’ll contribute:
Responsible to ensure that all referrals all entered into Current software system after receiving intake, pulling appropriate documents from Cerner, Epic, or from the physician’s office. Doing insurance verification to assure that there is not another home care agency involved. Professional communication with: case managers, home care coordinators, physician’s office regarding taking referrals and follow-up phone calls to inform of acceptance of referral. Once patient is entered and approved for service scan all appropriate documents into the system and provide the Clinical Supervisor with an original copy. Assist with answering incoming phone calls and distribute to the appropriate personnel. Attends meetings including but not limited to; staff meetings, in-services, case conferences, educational classes, office meetings and/or organizational meetings. Continued employment with the organization is contingent upon your ability to perform job duties in this job description.

The expertise and experiences you’ll need to succeed:
Minimum qualifications :
· High School Diploma
· Valid Driver’s License
Preferred qualifications:
· One (1) year of medical office experience preferably in home health.
· Skilled in use of the Current software system
· Associate’s degree
