Key Responsibilities
Accurately code and abstract both inpatient and outpatient medical records in line with ICD-10-CM coding rules and regulations.
Code all inpatient records within 5 days of discharge, ensuring a 95% accuracy rate, and interim code as needed if records or critical documentation are unavailable.
Code all outpatient records by the third day after discharge, including Ancillary and Wound Care services, adhering to ICD-10-CM and CPT-4 guidelines with a 95% accuracy rate.
Collaborate with physicians to resolve any ambiguities or deficiencies in documentation that affect coding accuracy.
Take proactive steps to ensure that accounts are correctly processed into the billing system and promptly report any issues that might delay the coding or billing process.
Utilize HMS to support accurate coding and ensure clean claims processing, ensuring all necessary documentation is present and entered correctly into the system.
Monitor billing error reports to confirm timely and accurate account processing and keep the Director informed of any unusual findings.
Abstract relevant physician information as needed to ensure accurate physician index reports.
Qualifications
High School diploma or equivalent required; Certification in ICD-10-CM coding (CIC, CPC, CCS) or a degree in RHIT is essential.
Prior Experience In Inpatient Rehab Coding Is Required.
Strong knowledge of ICD-10-CM coding, disease processes, medical terminology, anatomy and physiology, pharmacology, and laboratory terminology.
Ability to stay current with coding changes and updates.
Excellent communication, organizational, and numerical filing skills.
Proficiency in typing and computer data entry.
Ability to maintain confidentiality, demonstrate initiative and sound decision-making, and work independently.
Collaborative attitude and willingness to work effectively with physicians and other staff members.
