JOB DESCRIPTION:
The Director of Social Services will report to the Director of Integrated Behavioral Health and Social Services and will be responsible for providing operational and supervisory leadership to the Health Network’s social service teams. They will also monitor performance measures for each social service program quality outcomes and will work closely with Heath Network leadership to ensure the operational success of social service programming. In this role, the Director will foster collaboration and coordination between these teams, PHMC health center staff, and the PHMC continuum of services. They will also provide direct supervision to social service program managers, supervisors, as well to the health centers Social Workers. Additionally, the Director for Social Services will work directly with diverse and underserved patient populations and will provide community-based clinical social work as well as brief interventions to help coordinate care and improve patient health outcomes.

ESSENTIAL DUTIES AND RESPONSIBILITIES:
• Provide programmatic oversight, supervision, and leadership to social service program supervisors, program managers, and health center Social Workers (approximately 10 FTE).
• Examples of Health Network social service programs include, but are not limited to, HIV Medical Case Management, PrEP Navigation, Benefits Coordination, Community-based Care Management, and Peer Recovery services.
• Ensure the roles and workflows of each social service program are clearly and consistently communicated and implemented across the PHMC health centers.
• Ensure the operational success of Health Network’s social service programs.
• Ensure Health Network social service programs meet all performance measure goals and remain in compliance with funder requirements.
• Guide social service program managers and supervisors in developing workflows and best practices that are patient-centered and quality-based.
• Serve as the primary contact for external funders of Health Network social service programs.
• Continually communicate with health center staff, providers, and funders about social service program outcomes, progress, and results.
• Provide multidisciplinary assessments to patients to determine needs and barriers in health care and social supports, and access to behavioral health services.
• Collaborate with medical and auxiliary providers in developing health care plans and goals based on identified needs.
• Provide individual brief counseling including:
• Functional and strength-based assessment and diagnosis.
• Psychoeducation for patients and their support systems.
• Medication adherence counseling and disease self-management counseling.
• Motivational Interviewing to develop behavioral strategies aimed at symptom reduction.
• Brief problem-solving interventions aimed at modifying negative thinking and promoting self efficacy.
• Document all services in a timely manner in the electronic health record system.
• Develop and maintain new partnerships within the PHMC enterprise and larger community to better meet patient needs.
• Organize and facilitate ongoing trainings both for specific social service programs and for the larger Health Network staff, as necessary.
• Ensure social service staff remain up to date on all required certifications and trainings, necessary to perform their duties.
• Attend Health Network and PHMC leadership meetings, as required.

EDUCATION REQUIREMENT:
• Licensed behavioral healthcare provider required.
• Master’s degree in a social services field required.
• 2+ years’ experience in a supervisory role required.
PHYSICAL DEMANDS:
• Travel is occasionally required by personal automobile or public transportation.
• Walking from public transportation is required.
• Stair climbing may be required.
• Position may additionally require standing, walking, sitting, use of hands, reach with hands and arms, and talking and listening in-person and virtually.
WORK ENVIRONMENT:
• Moderate noise (examples: business office with computers and printers, light traffic).
• Shared work environment.
• Primary work environment is a medical facility, with a degree of exposure to tuberculosis and other air-borne disease that may require the use of personal protective equipment.
