The Medical Record Technician (MRT) Coder reviews the patient(s) health record and assign codes base on the provider clinical documentation for the date of service rendered. MRT Coder is experienced with using the electronic encoder and coding resources: Internal Classification of Diseases, Tenth Revision, Clinical Modification (ICD-10 CM), International Classification of Disease, Tenth Revision… Procedure Classification System (ICD-10 PCS), Current Procedural Terminology (CPT), Healthcare Common Procedure Coding System (HCPCS), Diagnostic and Statistical Manual of Mental Disorders (DSM), and/or Evaluation and Management (E/M) Level, etc. to the highest level of specificity based on the providers clinical documentation within the health record.MRT Coders are able to assist providers with clinical documentation requirements to completely and accurately reflect the patient care for services rendered; able to generate query verifications to providers when diagnosis and/or procedural clarification is needed; provides technical support in the areas regulations and policies, coding requirements, documentation guidelines, reimbursement workload, accepted nomenclature, proper diagnosis and procedure code sequencing, modifiers assignments, etc.MRT Coders are experienced with applying the current Official Inpatient Coding Guidelines when abstracting and accurately reporting the principal diagnosis, secondary diagnoses, principal procedure, and all secondary procedures. They are able to apply knowledge and experience with the use of the 1995 Documentation Guidelines for Evaluations and Management Services and 1997 Documentation Guidelines for Evaluation and Management Services.
